Definitely scary. I like how the focus is right on the inside of the mouth... leads the eye right in there.

Also, what's wrong with the composition? (I'm scratching my head at that comment.)

This makes wanna take a deep breath and feel happy that I can breathe. After studying it, I wonder what it would do if the subject were shifted more to the right upper corner with his head reaching back like his whole body is straining for a breath.

I dont like the subject in any way. Just sensationalism . Technically, the BW is good and the comp interestig althouh i would avec taken the shot closer

Composition is a bit off. I agree it needs to be shifted a bit. Also the texture in the mouth is different enough from the rest of the face, as to be distracting. Otherwise, you nailed the theme, SCARY!

Scary is all about black so i would have cropped the rigth of is face and just under is chin. you want scarier just add a black spider on his lip ready to go in or coming out of the cavern of is mouth,
this is what i would call a scene from an horror movies.
Good performance of your subject must have taken him effort to draw that plastic film in is mouth.
And you to take the photo before he suffocatted.
P.S. as for sensationalism it is to bring powerfull emotion good or bad, as art in any form and subject, try to do it.

An excellent interpretation of the theme, straight out of a horror movie, so fantastic job on going through the effort of setting up this shot to facilitate an idea.

If i were to suggest anything, I'd say that perhaps the impact of the photo could be strengthened by increasing the contrast of the image.

The highlights already appear to be close to white, but maybe pushing more of the shadows towards black to bring out the terror in the subjects eyes. And I agree that the impact might benefit from rotating the positioning a little counter-clockwise, placing the subject more towards the (implied) submissive lying down position.
